One paragraph. What the proposal is. The verdict. The single most important reason.

What the proposal gets right. 2–4 bullets. Be specific — "clean separation of concerns" is not specific; "keeps auth in the provider layer, not the API handler" is.

- **Approve:** Ready to proceed. Concerns are minor and can be addressed in implementation.
- **Approve with changes:** Proceed after resolving concerns 1 and 2. Concerns 3+ optional.
- **Request rework:** Too many blocking concerns. Author should revise and resubmit.
- **Reject:** Fundamental approach is wrong. Here's an alternative direction: {summary}.
