Systematically collect symptom information, generate a prioritized differential list, and produce a triage recommendation with clear urgency classification.
Systematically collect symptom information, generate a prioritized differential list, and produce a triage recommendation with clear urgency classification.
Adopted by: AVMA, ASPCA Poison Control, veterinary telehealth platforms, emergency veterinary triage protocols worldwide
Impact: Structured triage reduces delayed-care incidents; owner education on emergency warning signs reduces pet mortality from preventable delays — gastric dilatation-volvulus (GDV) mortality drops from 50% to <15% when treated within 2 hours of onset; urethral obstruction in male cats is fatal within 24–48 hours without intervention (Merck Veterinary Manual)
Why best: Systematic symptom collection prevents anchoring on a single sign, surfaces red-flag combinations that require immediate care, and separates true emergencies from wait-and-see presentations — reducing both under- and over-utilization of emergency veterinary services.

Collect species, breed, age, and sex/neuter status — these factors change pre-test probability for all differentials (e.g., GDV in deep-chested breeds, FLUTD in male cats, parvovirus in unvaccinated puppies).
Define the chief complaint precisely — elicit onset (acute vs. gradual), duration, progression (improving, stable, worsening), and frequency.
Screen for immediate red-flag signs — check for any of: difficulty breathing, collapse or extreme weakness, uncontrolled bleeding, suspected toxin ingestion, inability to urinate (especially male cats/dogs), seizures, suspected spinal trauma, pale/white/blue gums → any positive = EMERGENCY, advise immediate veterinary visit.
Collect full symptom history — appetite, water intake, urination, defecation (color, consistency, blood), vomiting (frequency, content, blood), activity level, and any environmental changes (new food, toxin exposure, travel).
Assess vaccination and parasite prevention status — unvaccinated status elevates probability for parvovirus, distemper, panleukopenia.
Generate a top-3 differential list — rank the three most likely causes by prior probability given species, age, breed, and symptom pattern; include one life-threatening differential even if less likely.
Classify urgency — assign one of four categories: (a) Emergency — seek care now, (b) Urgent — within 24 hours, (c) Non-urgent — next available appointment, (d) Home monitoring with listed re-evaluation criteria.
Provide interim home-care guidance — for non-emergency cases only: specific, safe instructions (e.g., withhold food for 6–8 hours for mild vomiting in an adult dog; never fast cats for more than 12 hours); state clearly what is NOT safe to give without veterinary guidance (human NSAIDs, Tylenol, Xylitol-containing products).
List "call-back" triggers — state the specific signs that should prompt immediate re-evaluation even if currently non-urgent.
Recommend veterinary follow-up — provide clear guidance on whether a primary care vet, urgent care clinic, or 24-hour emergency hospital is appropriate based on urgency classification and time of day.
