Show the current Claude Code session's plan as a clickable link in the status line. Bundles a PostToolUse hook that maps session_id to plan path, a status line script that renders an OSC 8 hyperlink (vscode://, cursor://, zed:// configurable) and can wrap an existing status line, and a setup skill that wires settings.json non-destructively.
