meta-edit

Replaces raw Edit / Write / MultiEdit with twenty-one kind-specific edit_* MCP tools (14 SQLite-derived impl tools + edit_cosmetic + 6 workflow-axis kinds) that encode the testing obligations for each kind of change directly in their tool descriptions. Every declaration carries a required provenance field naming the epistemic source of the edit. Impl tools carry a required prod/test target flag so test edits are visible inside their kind's audit surface.
