Goal-writing guidelines

• All goals must be SMART: Specific, Measurable, Achievable, Relevant, Time-bound
• Goals must be functional — tied to occupational performance, not just impairment measures
• Include the assist level expected (Independent, Supervised, Min A, etc.)
• Include a timeframe for achievement
• Short-term goals should be stepping stones to long-term goals

Important guidelines

• Tie interventions to specific goals and functional outcomes
• Include precautions prominently — safety is paramount
• Document prior level of function to establish a meaningful baseline for goal-setting
